To request medical
records, please call the Health Information Management Department at
641-842-1478. To protect our patient's privacy, medical records cannot be faxed
directly to the patients. Patients will be required to pick up their records in
person from the Health Information Management Department and sign a release
form. This is to protect our patient's privacy and security.
