  • Glucose monitoring

  • Follow-up care
Program Goal: Diabetes control will be achieved through teaching patients healthy habits for living well and coping with diabetes. Upon entering the diabetes self-management program, you will receive the following:
  • Individualized teaching by a Certified Diabetes Educator and Registered Dietician
  • Classes relating to diabetes self-management
  • Regular follow-up to facilitate optimal glucose control


A referral from your physician is required to participate in this program. This service is covered by most insurance carriers. Please contact your carrier regarding your individual policy. For more information, please contact Denise Boozell at 641-842-2151, ext. 1341.
