Program Goal:
Diabetes control will be achieved through teaching patients healthy habits for
living well and coping with diabetes. Upon entering the diabetes
self-management program, you will receive the following:
- Individualized
teaching by a Certified Diabetes Educator and Registered Dietician
- Classes relating
to diabetes self-management
- Regular
follow-up to facilitate optimal glucose control
A
referral from your physician is required to participate in this program. This
service is covered by most insurance carriers. Please contact your carrier
